What a notice must include
A written notice should include a physical or electronic signature; identification of the copyrighted work; identification and location of the material claimed to infringe; contact information for the complaining party; a good-faith statement that the disputed use is not authorized; and a statement, under penalty of perjury, that the notice is accurate and the sender is authorized to act for the copyright owner.
Counter-notices and repeat infringement
